Park Hill clinic, located on the corner of 33rd and Daliah, has been a part of the Denver community for 35 years. In that time, we have grown to serve a diverse population of more than 7,000 patients of all ages near the northern part of Denver’s Park Hill neighborhood. 

The center is certified as a Patient-Centered Medical Home, which means our team makes sure patients are actively guided through their care. Park Hill clinic also partners with the Colorado Black Health Collaborative, an organization committed to providing black communities with resources for sustaining optimal health and wellbeing; the Mental Health Center of Denver, Dahlia Campus for Health and Wellness, whose mission is to enrich lives and minds by focusing on strengths and well-being, by spreading the word about all their programs available to the community. Multiple RTD bus routes surround the clinic, with frequent stops along Martin Luther King, Jr. Boulevard; and the Colorado Department of Public Health and Environment (CDPHE) to establish partnerships with Community-Based Organizations serving African American parents with children 5 to 11 years old to educate them on vaccinations.
