Transitions and Recovery

Denver Health plays a central role in helping patients transition from hospital to home, so you can eventually return to normal life. Facilitated by a team of experts including Nurse Practitioners, Registered Nurses, Exercise Physiologists and Cardiologists , we play a hands-on role in working with you every step of the way to ensure that each part of your recovery is closely monitored for success.


Our Approach

Our Healthy Hearts Program enables patients and their families to manage daily living with heart conditions by teaching them about every aspect of care, including:

  • Informative education as a part of any issue related to congestive heart failure, heart attack, coronary stent placement, coronary artery bypass surgery, valve surgery or cardiac device placement. 
  • Cardiac medications, daily weight monitoring, diet counseling, exercise and smoking cessation. 
  • Follow-up monitoring

Before an eligible patient goes home from the hospital, we schedule a return visit within two weeks to review your progress with a Nurse Practitioner or a Registered Nurse and a Cardiologist. We also facilitate enrollment in the Colorado “Quitline” program for smoking cessation, and enroll patients in our Cardiac Rehabilitation exercise program.


Our Winning Track Record of Success

We’re proud of our Heart and Vascular team for their recognition by the American Heart Association (AHA) for advanced achievement in the prevention and treatment of heart failure. Healthy Hearts follows the recommendations of the AHA for guideline-based care for congestive heart failure and heart attack patients. The AHA has recognized our Healthy Hearts program with its Gold Award from 2009-12 and 2015-16 for excellence in the “Heart Failure Get with the Guidelines” program.
