E-Consent for School-Based Pediatrics

Register for the 2024/2025 School Year

To register yourself or your student for next year, please complete one of the consents below. 

This consent is good for 4 years, or until patient no longer meets SBHC criteria.

 

How to Register

Our services are conveniently located at 19 different sites all throughout Denver. If you would like your student to be able to use the SBHCs, please complete the consent form below.

Parents or guardians must sign consent forms before a student may use most of our services. Once this is complete, students may use the health center at any time during the school year. Consents are good for one school year.

Note: In accordance with Colorado State Law, Denver Health School-Based Health Centers allow students (if requested) to consent themselves into certain confidential services. Confidential services include both behavioral health and reproductive/sexual health services. Parental or guardian consent will still be required for the use of all other services for students under the age of 18.  Additional background information can be found here.  Any questions from parents/guardians can be directed to School-Based Health Centers at sbhc@dhha.org or 303-602-8958.
